Purchasing custom made frames is much like a mtm shirt. They will show you tiles of different materials, basic frame shapes which can be modified to your liking and a selection of hardware. Once those have been selected they will take measurements such as bridge width, temple length, pupil distance, which will be used to determine the frame dimensions. After that they will usually give you some choices with the lenses, such as tint and material, a good optician will help direct you to what is right for your needs. Some places may schedule frame fittings, they make a mock up plastic frame to check the fit before they begin the final frame, where as others will go straight into production of the final frame. The whole process usually takes 6-8 weeks.
Facial Index on Grand (across the street from the Yohji store) is a great optical that carries many japanese brands.
