Shucks, I'm a 42 in RO (not tight; a finger's spacing in between) and generally need a 26 for most homme plus footwear. that said, remember that homme plus sizing is extemely inconsistent - i've owned shoes varying from size 25 to 27 - so make sure the webstore has a good return policy before you commit.

Most japanese brands are sized on the small side but I'd say yes...I'd expect the med. to fit like a small or xs and so on.
