I would assume not, but shoulder and pit measurements read more like a 46-48 so I'm confused / hopeful.

Mostly you're going to have to look at designers that use leather as well as faux. Comme uses faux quite a lot for biker jackets, for instance.

As new, probably yes, I doubt you can find an identical replacement. they are pretty bad insoles imo, while they feel quite comfortable, they don't breathe at all nor do they have any support.

You can buy them at any big drugstore. The quality will depend upon how much you want to spend. The most expensive are usually $20ish, which ought to be no problem for a guy who buys CCP derbies.
